What financial information should I bring to my appointment?: Please Bring:
o Proof of Income: This would include current pay check stub, completed wage verification form, or other income documentation for ALL People living in your household. If self-employed, you may bring your most recent completed tax return.
o If anyone living with you is currently on Public Assistance, Public Housing, Section 8, Food Stamps, Unemployment Benefits, or is receiving Child Support, you must provide verification of these benefits.
* All insurance cards, including Medicaid and Medicare, Be Smart (Family Planning Medicaid), or NC Health Choice

Health Department - Eligibility

1. What method of payments do you accept?: Cash, Checks (no starter checks), Money Orders, Visa, MasterCard, and Discover.

2. Who do I make checks payable to?: Make checks or money orders payable to: Iredell County Health Department or I.C.H.D.

3. What financial information should I bring to my appointment?: Please Bring:
